Period cramps are usually worst on the first day due to higher levels of Prostaglandins, which trigger strong uterine contractions to shed the lining. These intense contractions can reduce blood flow temporarily, causing pain and discomfort. As the period progresses, prostaglandin levels decrease, leading to milder cramps by the third or fourth day.

Factors like stress, hormonal imbalance, or conditions such as Dysmenorrhea can make cramps more severe. Proper hydration, light exercise, and pain relief methods can help manage discomfort and improve overall menstrual health.
